  3. Provide appointment slip; will notify via text or call for confirmation of appointment

Household validation of potential shelter beneficiaries in the 36 high risks coastal barangays

Clients:Residents from the 36 coastal barangays and those living in hazard zones

  1. Submit client’s primary data –(e.g. name, address, spouse,number of dependents)
  2. Search client’s records in the database
  3. Provide the client with the database search result
  4. Interview/validate the client by assisting the client to accomplished the Application Form
  5. Prepare a client name tag
  6. Take a photo of the client with the name tag
  7. Check the accomplished and signed Application Form to theclient

Verification of qualified shelter beneficiaries

Clients:Validated Households, Barangay Officials, other concerned offices

  1. Submit client’s primary data – name, address, spouse, numberof dependents

  3. Provide the client with the database search result
  4. Prepare and/or provide master list of qualified shelter beneficiaries
  5. Prepare house tag cards for qualified shelter beneficiaries

Request for housing assistance

Clients:Inhabitants of Tacloban City

  1. Submit client’s primary data – name, address, spouse, numberof dependents
  2. Assess the client’s information
  3. Provide the clients with the various type of housing project
  4. Ask for client’s additional information based on the type of housing project
  5. Provide referral to concern personnel for appropriate action
